There are 37 set questions that can be answered in this consultation. You do not have to respond to every question. You can also enter any additional comments directly into the 'Provide general feedback' section and have the option of uploading documentation to support your submission (one PDF). We welcome all comments and submissions.
You are required to enter your details and complete the consent to release questions before submitting your feedback.
How to fill out this survey
- There are five required questions under 'Submitter details'. The rest of the questions are optional; you can answer as many or as few as you'd like.
- If you want to make a general comment, we invite you to do so under 'Provide general feedback.' You can also upload a PDF to support your submission there.
- You can save your progress and come back later - you'll be sent an email with a unique Response ID and a link to return to the survey.
- The 'Finish' button will appear in the bottom right of this page after you have answered all of the required questions.
- Once you click 'Finish,' there is one opportunity to return to the survey. You cannot go back and change your answers after you submit.
